i just had Mike from Designing Renovations in to pull up my entire kitchen floor (down to the joists) and lay new subfloor. he even showed me how to install my new floor (and ended up doing most of it), helped with a pipe that was giving me trouble and installed my gas stove (shhh, i don't think he had the proper license to do that,
fyi
).
i paid $600 plus materials and he and his assistant (who's name i can't remember, i feel horrible about that!) came on a sunday and did it all in one day. considering i called him friday morning i was quite pleased with the timetable.
here's his website:

another suggestion is to check angies list. you have to pay to join but i have found many great great contractors through them.
i will say, i have probaby paid more now than the original quote i got to do everythign form demo to install, but i've learned so much doing it myself, including how to survive without a kitchen for 6 months
