I just turned over to Kitchen Renovations on DIY Network - the episode is Urban Conversions (2 of 4): Flat-Pack Cabinet Assembly and the cabinets are IKEA Hallarum cabinets! They show making a refrigerator surround wall with pantry cabinets on one side and because of a column, stacking wall cabinets that are bumped out to make them even at the front.
You can see the blocks they put behind the wall cabinets on the right to bump them out. The refrigerator will go in the opening between the two sets of cabinets.

They have a good list of tools and some good pictures of assembling the cabinets in the article.

I saw it the other night too ... favorite part was something along the likes of "The architect designed a kitchen that was going to cost us $30,000. So we took the layout to Ikea and got a similar design for $3,000."

I liked that they showed ways to use the cabinets that are not in the catalog. We all know IKEA cabinets can be used differently but not everyone can think "out of the box".
